We are out of network for all other private insurances at this time. Per request, we can provide a document showing proof of service with procedure and diagnostic codes (also known as a Superbill) for you to submit to your insurance company. Please check with your insurance company and HSA (Health Savings Account) for specific coverage and reimbursement policies. Many families find they can fund therapy through their HSA * Therapy practices are in accordance with current HIPAA requirements.
Payment for assessment and treatment sessions are due at the time of service. Payment is accepted in the form of cash, check, or credit card. For a complete outline or private payment fees, please contact us at firstname.lastname@example.org.
Credit Card Policy:
All clients will complete a credit card permission form. A credit card will be kept on file for all clients for charging of individual sessions if parents would like. A credit card will also be utilized in case of no shows and to be charged weekly (with parents’ permission). If credit card is declined, sessions will be held until sessions are paid in full.
Cancellation/No Show Policy:
All appointments that are missed without notification 24 hours before will be charged $100 for the missed appointment. Patient will not be seen if late cancellation fee has not been paid.
